If this was in game, I'd say they wouldn't walk until 2, would make mothers slowing picking them up and would be slower as a child than normal, but faster as an adult than normal, they could also pick up people until they're nine (If normal) They would also take longer to die from wound, but, shorter to die from starvation.

As has been stated with every one of these birth defect suggestions is; why would you not just suicide immediately after being born with a birth defect? What would be the point of intentionally handicapping myself for an hour?

People wouldn't suicide if it was beneficial to them to live through hardship. Like some extra bonus to genetic fitness or whatnot.
Children with harmful disabilities die more often? Well that's how it is like in nature.
Might be interesting to have a life 'different' but perhaps it could get gimmicky fast.

It's not exactly a defect, there are some things that give players with gigantism an advantage

Double fertility but also double pip drain.
Move faster, move normally with heavy items but extremely low character limit.
Understand everybody, start with the max character limit, Everyone is forced following you but you cannot learn any tools.
Unlimited tool slots, Enter all biomes but you cannot speak and can't understand anybody.
You do not need to eat but you die at 30.

but jason draws characters manually so each sprite would need to be redrawn for combinations

Not everyone plays the game as a survival simulator. A good chunk of the playerbase is in it for the roleplay, for meeting others, for living cool stories. And having a disability just so happens to be a cool, unusual story. You'd get attention from others, you'd need to come up with solutions to live as you are, etc. Being different is fun.
It has no beneficial purpose, and I am against it. However, your questions aren't that useful, as you're just going off of how you play the game, not how others do.
