We have yellow fever, how about, sickness caused by lack of hygiene? locus swarms eating crops (starvation insted of disease)
and lack of hygiene, well, there could be three symptons, 1. hungry or less pips, unlike yellow fever, you can actually eat. 2. slowness and less screen room, this is actually a pretty easy thing to add. and finally, 3 a mushroom like disease, causing illusion, like, making you make people with weapons tryna kill u when they aint, some sprites (not all) changed, and it could get worse depending if your hygiene gets worse, welp, GOTTA GET DAT SOAP (or medicine D: ): )

Communicable diseases are obviously interesting from a sim point of view.... almost an "in the name of science" kind of thing.
They also might be the source of an interesting social gameplay challenge. Eradication might be possible, in the hands of the players, for example.
However, I suspect that it would swamp all other aspects of the game, so it's probably out of scope.

You said you might add it (you did well update and homeland sick no bbs update instead) maybe this could be a good chance to replace racial restrictions, I mean, you could be slower and get diseases in foriegn lands! instead of no enter cos racism!

I wouldn't mind a disease lingering around that would kill me in, say, 30 years. It could require the decision of one guy to actually invest some of his time to fix it for the whole family by creating medicine.
It would always be a thrill since you constantly feel like running out of time to save the next beloved guy on death row. I find that motivating, actually (same thrill when trying to fix someones wound, but with more time for drama and also more complexity).
Also, this could be one way to reduce population density in big cities. The last update took all my motivation for connecting people as every abandoned well is practically useless - making me wish to never have connected with it in the first place.

Diseases shouldn't kill. But maybe make temporary slowness, high temperature, make unable to ride a horse and maybe unable to use tools (only tools, not all items)... Just temporarily, like 4 minutes maybe.
Here is an idea how could it work:
Eating unhealthy (mono diet) could have a small chance to make you sick after eating another non yum food... Eating non yum food would have 0% chance to make you sick if your yum was at least +5 before eating non yum food and the chance to make you sick would increase the less yum you had before eating a non yum food (let's say 15% if you had no yum bonus at all, 12% if you had +1 yum bonus and so on)
As for medication we would have vaccinations. Once a person gets vaccinated, he/she would never get sick no matter how unhealthy he/she eats.
That's not really a good idea but it came to my mind and I wanted to share

Perhaps not diseases, but just afflictions in general. Slowness caused by pains, curable using aspirin from willow trees, is something I've been wanting to see

For example: what if we had to bury bodies and keep dung in buckets away from food and ourselves because we might become sick?
Graveyards would have an actual purpose, players would be more organized, civilization more civilized, you get the message.

I guess the problem is that we use the same bowl for dirt, poisonous liquids, and food of both cooked and uncooked variants. But I guess that's just nitpicking without actually improving the gameplay experience.
