Transforming skins?

I don't know if this is a bug or just my terrible laptop, but sometimes the skins of items and people swap with other things. For example, I recently turned into a bear cave. A plant nearby me turned into a couple of ducks.

This has happened to me two or three times now, and this time it snapped back after a minute. Another time it was far more severe and everything was turning into different stuff, to the point where I couldn't even play. There's no specific action that I take (I believe) that triggers this. Has anyone else had any experience with something like this?

Oh, well, yes, I did. This time at least. The other times I don't recall having done that.

That's the only thing that would do that. If you die while under the effects of the mushroom the effect stay on until you restart the client. Maybe that's what happened to you

You can eat a mushroom again to reset it.
