Since all exiled followers are excluded from inheriting leadership
What would happen if the leader exiles all followers (has to be lord or lady unless lord or lady under them did the same) and then dies? would the civilization go into leaderless anarchy?

When a leaderless leader dies, and we pick their fittest follower to take over as leader, we now ignore exiled followers (unless we have no choice, and all followers are exiled). Fixes #708
